APPLICATION FOR “FROM FAILURE TO PROMISE” ESSAY SCHOLARSHIP.


Please read the following instructions carefully, failure to follow the instructions may make your application ineligible:


1. All applicants must be enrolled or planning to enroll into an accredited college or university in the Fall of 2011. (High school seniors and current college students are both encouraged to apply)

2. Applicants must have a Cumulative GPA of at least 3.0.

Scholarship monies will be paid directly to the student.

JOB DESCRIPTION 

Title: Diabetes Project Student Assistant
Exemption Status: Non-Exempt 
Office Location: Sacramento 

Part-Time: 10 hours per week 

Position Summary: 
•Under supervision of the Diabetes Project Director, provides clerical and administrative support to the Diabetes Project 

Duties/Responsibilities: 
•Type, proofread, and edit all types of correspondence 
•Take telephone calls and messages 
•Copying, filing, mailing, faxing 
•Maintaining records and databases 
•Organizing and duplicating materials for meetings and conferences 
•Appointments scheduling 

Contacts: Regular contact with CMA Foundation Board and staff, CMA staff, other foundations and nonprofit organizations, physicians and other health care professionals. 

Knowledge/Experience/

Education: 
•High school graduate 
•Strong organizational skills and attention to detail 
•Ability to work quickly and effectively under deadlines 
•Accurate typing 
•Word Processing proficiency 
•Good telephone manner 

Supervision: Reports to Diabetes Director 

Equipment Operation: 
•Microsoft Office: Access, Excel, Outlook, and Word 2007 
•Fax machine 
•Copy machine 
•Postage meter
